The TraumaSim Lifecast Male Upper Torso with Sucking Wounds is a high-fidelity chest trauma training aid designed for the demonstration and practice of open pneumothorax management — one of the most time-critical, life-threatening chest injuries a first responder or tactical medic will encounter.

An open pneumothorax — commonly referred to as a sucking chest wound — occurs when a penetrating injury creates a direct opening between the chest cavity and the outside air. Each breath draws air through the wound rather than into the lung, rapidly compromising respiratory function. Identifying it and sealing it correctly is a foundational TCCC and pre-hospital trauma skill. Training that response requires a trainer that delivers both the visual presentation and the physiological cues that make the injury recognisable under stress.

This trainer delivers both. Two anatomically positioned chest wounds — left and right sided — each feature independent blood and air delivery systems. Compressing the included syringe delivers bleeding from the wound site; pumping the hand bulb simulates air movement during respiration, creating visible bubbling at the wound that directly replicates the characteristic presentation of an active sucking chest wound. Trainees don't just see a wound — they see it behave like a wound.

Each wound system operates independently, allowing instructors to run bilateral presentations, isolate a single side, or vary bleeding and airflow intensity throughout the scenario to increase challenge or reward correct intervention. TraumaSim recommends Ben Nye Stage Blood diluted at 1 part blood to 3 parts water. A 500 ml bottle is included.

The torso is a male lifecast — anatomically accurate surface detail for realistic chest seal placement practice, including correct landmark identification and seal positioning. It's washable, durable, and built for high-volume repeated use.

What is a sucking chest wound and why does it require dedicated training?
A sucking chest wound — clinically an open pneumothorax — occurs when a penetrating injury creates a direct opening in the chest wall. During respiration, air moves through the wound rather than the airway, progressively collapsing lung function. It's immediately life-threatening and requires rapid identification and correct chest seal application to manage. The characteristic bubbling at the wound site is the key diagnostic cue — and that's exactly what this trainer replicates.

How does the bubbling simulation work?
Each wound is connected to a hand pump via tubing and a connector. Pumping the hand bulb forces air through the wound cavity, creating visible bubbling at the wound site — replicating the air movement characteristic of an active open pneumothorax during respiration. The bleeding syringe and air pump operate independently, so instructors can run both simultaneously or separately depending on the training scenario.
